Services |
|
There are
SIX major service components that have been identified as common
services required by all groups of people with disabilities. The
services that a person receives will be determined by their individual
needs, within the limitations of available resources. The selected
components of the services are then included by the NASC Provider
(Contracted assessors to the MOH), as part of a referral plan for each
client, and sent through to the preferred Provider Agency.

Personal Care
– is defined as assistance with activities such as:Bathing, Toileting, Drying/Dressing,
Assistance with transfers to commode/bed and or electric wheelchair,Feeding
-
Home Care –
is defined as services which enable a person to organise and control
their household environment and can include but are not limited to:
Laundry, Meal preparation/Cooking, Shopping, Light property maintenance

Carer Support -
a service response to provide relief to carers (previously Aid to
Families/Whanau); only applicable on specific referral from a NA/SC
Provider contracted by the MOH.

Day Care Programmes -
This an additional contract purchased by MOH which may be required by
NA/SC providers to meet the client’s assessed needs. Day Care
services can include a range of planned activities to provide
assistance with a mentally and socially stimulating programme with
recreational and therapeutic benefits to clients.

Personal Health funded Home Help -
providing care and assistance to the Client as requred with
activities such as preparing for bed, getting supper, being available
for 3 calls for assistance during the night; showering and dressing in
the morning and so on.

Mental Health funded Home Help -
assistance with household tasks only, as referred by NA/SC Provider

OBJECTIVES:

to maximise independence and contribute to those client’s
who wish to remain in the community

to enable people requiring assistance with activities of
daily living to remain safely in their own home environment for as long aspossible

to complement and enhance the support provided by
Family/Whanau, and the community networks
Needs excluded:
-
People who have severe psychiatric or behavioural problems only, i.e. without also
having a severe physical disability.
- People whose needs require procedures which the manager considers
should be carried out by qualified health
professionals.
